For hospitals

Continue your patients’ care at home

How referral works

One place for every referral

Refer online in minutes

Send the referral with the diagnosis, the urgency and the discharge summary through the secure AnidasoCare platform, from a computer or a phone.

Reviewed by a palliative care specialist

Every referral is reviewed by an AnidasoCare palliative care specialist, who accepts it, asks you for more information, or explains why home care is not the right fit.

Follow every referral

See where each referral stands, from review to the first contact with the family. Your dashboard shows how quickly AnidasoCare responds.

Hear back without chasing

You are notified in the platform, and by text if you choose, when we need more information or have made a decision.

Know how your patients are doing

With the patient’s consent, you see that they are in our care, how many home visits they have had, and how their care ends.

Secure by design

Each hospital sees only its own patients. Documents are stored privately and encrypted, every access is logged, and staff accounts can use two-factor sign-in.

For your oncologists

Referring a patient?

Oncologists have their own accounts. They register the patient, send the referral, answer the specialist’s questions and follow their patients from one dashboard.

Referrals are not for emergencies. A patient who needs urgent medical care should be seen in hospital.

Partnering with AnidasoCare

Bring home palliative care to your patients

Partner hospitals agree how patient information is shared, and their staff receive their own accounts on the AnidasoCare platform. Get in touch to discuss working together.
