For oncologists

Refer your patients for palliative care at home

When your patient needs help with pain and other symptoms at home, refer them to AnidasoCare online. A palliative care specialist reviews every referral, and you can follow what happens next.

Referring a patient

From your clinic to their home

  1. Step 1

    Your own account

The AnidasoCare team creates your account. At first sign-in you choose your password and add your Medical and Dental Council registration number and your hospital, which we check before you start.

  2. Step 2

    Send the referral

Register the patient, describe their illness and needs, choose how urgent the referral is and attach the discharge summary or clinic letter, from a computer or a phone.

  3. Step 3

    Answer questions quickly

If the specialist needs more, you are told exactly what is missing and can reply straight from your dashboard, without phone calls or chasing.

  4. Step 4

    Hear the decision

A palliative care specialist decides whether to accept the referral, decline it or suggest another service, and the reasons are recorded on the referral for you to read.

Who to refer

Patients who would benefit from care at home

Refer patients living with cancer whose pain, other symptoms or needs at home would be helped by palliative care, at any stage of their illness and alongside any treatment you are giving.

Mark how urgent the referral is (routine, priority or urgent) and explain why. The specialist uses it to decide who is seen first.

Referrals are not for emergencies. A patient who needs urgent medical care should be seen in hospital.

After the referral

Stay informed about your patients

Once care begins, the patient decides what is shared with you. We ask them, or the family member who decides for them, and they can change their mind at any time.

Every nurse’s visit report is checked against a set of clinical warning signs, such as low oxygen levels or a sharp rise in pain. Urgent signs alert our palliative care specialists straight away, and you too if the patient has agreed. Our specialists respond; the alert keeps you informed.

Your dashboard

Everything about your referrals in one place

Your referrals at a glance

Referrals sent, accepted and waiting, where each one stands, and which need a reply from you.

How quickly we respond

Time to a decision and to first contact with the family, measured against response targets for each level of urgency.

Your patients in our care

Who is receiving home care now and how care ended, for patients who agreed to share their progress with you.

Alerts when something changes

You are told in the platform, and by text unless you turn texts off, when we need information, have decided or have started care, and, if the patient agrees, when an urgent concern is raised at home.

You see only the patients you referred or were asked to follow, at your own hospital. Colleagues do not see each other’s patients, every time a record is opened it is logged, and texts never include patient names or medical details.
